Working with Medicaid Managed Care Plans, the members of PACH have all contracted with local agencies. These CBOs employ Community Health Workers who work with individuals in the community, conducting risk assessments and connect them to care and services.

PACH members have significant expertise in convening community partners, payers, and providers to align on health outcome related goals. Through learning session, conferences, and webinars - the team at PACH leads to create forward momentum in outcomes for our neighbors.

Each member of PACH has programming to innovate in creation connections amongst partners. These innovations have included addiction hotlines, diversion court programming, housing voucher programs, breast cancer screenings, and more.
